Please note: Pre-payment of fees is required in all cases. We accept cash or cheque (accompanied by valid guarantee card). We are unable to give credit or accept credit or debit cards. If a patient fails to attend for a private appointment, a cancellation fee of 50% for the service requested is charged.
Accidents:
Consultation following RTA per person - £40.00
Certificates:
Simple Certificate (single statement only) - £15.00
More complex - £25.00 to £50.00
Copies of Records, Letter, etc:
Computer generated only - £15.00 to £50.00 (depending on quantity)
Paper or mix of computer and paper - £50.00
Employment:
Medical (examination & report) - £125.00
Report/Questionnaire only - £56.50 to 113.00 (depending on length & time taken to complete)
Holidays:
Cancellation or fit to travel certificate - £25.00 to £35.00
Immunisation Hep B Package, course of 3 - £144.00 (only available to patients not registered at this surgery)
Certificate of Immunisation - £15.00
Examination - £95.00
Insurance:
General Practitioner Report (eGPR) - £84.00
